The Social Services Director is responsible for ensuring the residents of Bonnie Bluejacket Memorial Nursing Home and, as necessary, the patients of South Big Horn Critical Access Hospital receive and maintain the highest practicable physical, mental and psychosocial wellbeing. The Social Services Director will guide facility staff in matters of resident advocacy, protection and promotion of resident rights. This position will maintain compliance with state and federal regulations and ensure the highest standards of professional social work practices are met.


Education:       Bachelor's degree in social work or bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling or psychology from an accredited institution.

Licensure:        N/A

Work Experience: Minimum I-year supervised social services experience in a health care setting working directly with individuals. Long term care experience preferred.

Freedom from illegal use of drugs, and freedom from use and effects of use of drugs and alcohol in the workplace.

Persons who have been found guilty by a court of law of abusing, neglecting or mistreating individuals in a health care related setting are ineligible for employment in this position.


  • Knowledge of Medicare, Medicaid and Wyoming skilled nursing facility rules and regulations.

Possess strong verbal and written communication skills  Creating a safe, welcoming and efficient environment

  • Experience with electronic medical records
  • Basic Computer knowledge

Promote facility mission, vision and values through daily work assignments; communications with co-workers, patients and families; practice accountability in work product; and advocate for higher quality of care.

Must possess a team-oriented attitude and adhere to a continued willingness to implement new systems of operation when changes are required to promote patient and facility health, safety and satisfaction.

Will engage every customer encounter with a high degree of service excellence focusing on creating a welcoming environment, anticipating customer needs and providing excellent communication at all levels of the encounter to help the customer feel at ease, informed and confident in the care received.

Ability to work independently and manage multiple projects and activities simultaneously. Must be able to prioritize projects and handle change productively when assigned additional


Conduct self in a professional, courteous manner with co-workers, staff and the public in order to maintain a cooperative relationship among the healthcare teams. Must adhere to a philosophy and culture of teamwork and cooperation at all times.


Must assess and collect data to evaluate current records and pertinent social data concerning personal and family problems medically related to resident's illness and care shall be maintained in each resident's record.

Gather information and conduct preadmission screening of individuals seeking admission to the facility.

Completes and updates social service assessments for assigned residents/patients as part of the comprehensive resident assessment process. Assessments must identify residents with current needs for social service interventions to improve or maintain functional abilities and those residents at risk of psychosocial deterioration.

Interview residents, interested family members, legal representatives and significant others to obtain and update information needed to develop individualized plans of care to protect and promote residents' rights.

In consultation with the inter-department disciplinary team, will develop, implement, follow and monitor admission and discharge planning and resident care plans and the services associated with each resident.

Audits clinical records of residents on a regular basis to identify concerns about resident rights.

Provides social service visits as needed to attain or maintain the highest practicable mental and psychosocial well-being of residents.

Records mental and psychosocial progress notes in the clinical record no less than monthly.

Identifies ways to accommodate resident's choices, preferences and customary routines. Includes these approaches in plan of care.

Shall create, implement and maintain policies related to social services care and confidentiality of residents within the facility.

Assist with providing in-service training to facility staff directed toward understanding emotional problems and social needs of residents and the means of taking appropriate action in relation to them and the necessity of confidentiality.

Gives residents/patients, family members and legal representatives information about their rights to make choices about healthcare decisions and about eligibility for Medicare and Medicaid benefits.

Will act as a liaison with community resources for legal, financial, mortuary, social, health and communities' agencies and other services. Assists with applications for benefits and procurement of services, clothing, personal care items from community sources outside the facility.

With the consent of resident, maintains ongoing contact with family members and/or legal representatives regarding changes in condition, discharge planning efforts and to encourage family participation in developing plans of care.

Completes reporting requirements as required by government agencies, such as MDS submissions and PBJ reports.

Participate in quality assurance program within the skilled nursing facility.

Assists in the preparation and participation of State surveys during annual reviews and special cite visits.

Will assume job responsibilities and act in the absence of the nursing home administrator.

Other duties as assigned.

Essential functions are carried out in a variety of positions including standing, stooping, bending, squatting, lifting and/or sitting. Virtually all positions require the ability to move freely through the building.  May have long periods of time sitting.

  • Lift, carry, turn and manipulate items with a weight of 50 lbs. or more.
  • Exposure to infectious material.
  • Exposure to the stresses associated with an intimate, 24-hour care environment that delivers care and services to the disabled, cognitively impaired, and acutely ill and injured.

8 to 10-hour work shifts. May be required to come report on a day off due to serious social, physical or health events occurring within the facility.
