Improving the Transition from Hospital to Home

Sometimes it is necessary to require skilled nursing care after a hospitalization from an injury, surgery, or acute illness.

Three Rivers Health offers a Swing Bed Program, skilled care to help you regain strength and independence in a hospital setting while being close to home and having the support of your loved ones. We help you restore your health and reduce your chances of being readmitted to the hospital, so you can get back to living your life.

Our Services

Three Rivers Health Swing Bed Program is a combination of 24-hour nursing services, rehabilitation therapies and education, including:

  • Cardiac monitoring
  • IV Therapy
  • Patients needing to regain strength and mobility after an illness or surgery
  • Feeding tube adjustments
  • Complex wound care
  • Physical, occupational and speech therapies
  • Patient and family education for new conditions such as diabetes or colostomy care
  • Nutritional counseling
  • Patients needing diabetic management
  • Patients who need rehabilitation or therapy after orthopedic surgery such as joint replacement
  • Patients recovering from a stroke when you do not qualify for admission to an inpatient rehab center

Visitation with my Loved One

Patients need the support and encouragement of their friends and family, whom we invite to visit often. If you have a visitation request outside our normal visiting hours, special arrangements may be made by contacting the nursing staff.

Family members are encouraged to attend care conferences for their loved one.

What Happens After the Swing Bed Program?

Each patient makes progress based on individual medical needs and rehabilitative potential. The multidisciplinary team will continuously monitor your progress and evaluate new goals until they determine you are ready for discharge.

Care management will assist you with discharge and a therapist may accompany you to your residence to complete a home safety evaluation. Therapists can also educate you and your family or caregiver on safe body mechanic techniques and practical adjustments that can be made in your home or residential facility.

You may be referred to home health or rehabilitation services for additional therapies to help you attain your long-term recovery goals. This may include learning how to use adaptive devices and assistance equipment properly, learning exercise techniques you can use at home, and learning preventative measures that reduce risk of injury and falls.

Who’s on My Team?

The Swing Bed Program is managed by an expert group of caregivers under a doctor’s direction. Your care team may include the following:

  • Provider: Board certified providers care for each patient and are available 24 hours a day
  • Care Management: Usually the first contact a hospital, doctor or patient will have regarding a possible swing bed stay. Once you're admitted to the program, care management works with you and your family to connect you to services you need
  • Nursing Team: Skilled nurses to provided education, wound care, IV infusion therapies and will assist you in your journey to meet your recovery goals
  • Physical Therapist or Physical Therapy Assistant: These specialists help you with mobility and pain reduction using a variety of exercises
  • Occupational Therapist or Occupational Therapy Assistant: They help you meet goals for daily living activities such as dressing, bathing, cooking, eating, or maybe writing or using a computer
  • Speech Language Pathologist: Speech therapy may be needed for patients who have difficulty with swallowing and speech
  • Dietitian: Proper nutrition is important for recovery and good health. We offer nutritional support, education and, if necessary, custom diets to meet your preference and dietary needs

Who Qualifies for This Program?

In addition to getting your doctor’s referral, you must be in the hospital for at least three nights before transfer to the Swing Bed Program. If you have been discharged to your home from a hospital, but are having trouble recovering, you can still be admitted into our Swing Bed Program. You must have stayed in the hospital for at least three nights in a row within the last 30 days.

Talk with your doctor or care management who will refer you to our program. Once a referral is made, our Swing Bed Coordinator at Three Rivers Health will look at your case to see if our program is a good fit for you.

How is This Different from a Regular Hospital Stay?

With our Swing Bed Program. we'll help you get better and regain your highest level of independence. During the day, you'll get therapy and treatment based on your condition and recovery goals. Similar to the hospital, you'll receive around-the-clock nursing care overseen by a doctor.

Best of all, you will be able to trade the hospital gown for your own comfortable clothing.

What Does Insurance Cover?

Medicare and some insurance plans:

  • Cover the first 20 days of care at 100%, as long as there is an accepted skilled service needed, provided and accepted
  • Require a co-pay for 21 days through discharge (or day 100, whichever comes first), as long as you continue to require a swing bed. If you have supplemental insurance, it may cover your co-pay

How Long Can I Stay?

You can remain in our Swing Bed Program as long as you have therapy and rehabilitation goals to meet. Once you have reached these goals, you may be able to return home. If you meet your rehabilitation goals before 20 days, you may be discharged. You may also need to explore additional care options.
