POSITION SUMMARY:

The Rural Health Clinic (RHC) visiting nurse home visit will be performed by a registered nurse (RN) who will be accountable to provide skilled nursing care as prescribed by the physician, and in compliance with agency policy, to promote quality care with continuity to the individual in their home setting.  A designated registered nurse will accept supervisory responsibility for nursing services performed by a licensed practical nurse.  The RHC Clinic Manager will accept responsibility for overall administrative and supervisory functions.

MINIMUM QUALIFICATION STANDARDS:

Education:       Graduate of an accredited school of nursing

Licensure:        Current license as a Registered Nurse in the state of Wyoming; or current multistate Registered Nurse License and be in the process of changing home state licensure to Wyoming.

Work Experience:       Prior healthcare experience as a med surge nurse, home health nurse or similar positions preferred.

Free from illegal use of drugs, and free from the use and effects of drugs and alcohol in the workplace.

Persons who have been found guilty by a court of law of abusing, neglecting or mistreating individuals in a health care related setting are ineligible for employment in this position.

ESSENTIAL JOB FUNCTIONS:

Travel to patients’ homes and manage care plans according to physician instructions.

Maintain knowledge and technical skills to provide nursing care in the individual homes of clients.

Develop a good understanding of client’s diagnoses and medical regime and coordinate nursing care with medical care. Assisting, communicating, and cooperating with medical providers to give high quality care.

Record care plans and input accurate and appropriate documentation into patient charts.  

Perform the basic nursing process:  observation and assessment, nursing diagnosis, planning of patient care, implementation of care plans, and evaluation of results.

Coordinate patient care with other resources as necessary.  This may include nursing services, community services, and medical services.

Assure for the provision of pertinent health education to clients and their significant others.

Assess and evaluate physical, psychological, and emotional aspects of patients and provide appropriate therapies to patients based on the assessments with interdisciplinary team assistance.

Provide and facilitate diagnostic and other lab tests to patients.

Administer medication and insulin, and completing blood pressure, glucose, urine, and stool tests.

Inspect wounds, change dressing and handling personal grooming and hygiene.

Organize, prioritize, and schedule visits to homes, communities, and residents to deliver nursing care therapy.

Have knowledge of facility policies and procedures and a commitment to adhere to them, thus protecting the overall welfare of the patient and of this facility

Actively participate quality improvement of the department to provide quality care to all individuals who seek our services.

Work with nursing staff to identify discharge needs, assist with discharge planning and actual discharge.

Participate in weekly utilization review meeting and nurse report as necessary.

Assist with process improvement initiatives as they may relate specifically to visiting nurse services.

Observes all Federal, State, and local laws, rules, regulations and facility policies and procedures.

Perform other duties as assigned.

PHYSICAL, PSYCHOLOGICAL AND ERGONOMIC REQUIREMENTS:

  • Essential functions are carried out in a variety of positions including standing, stooping, bending, squatting, lifting and/or sitting.  Virtually all positions require the ability to move freely through the building. 
  • May have long periods of time standing.
  • Lift, carry, turn and manipulate items with a weight of 50 lbs. or more.
  • Exposure to infectious material.
  • Exposure to the stresses associated with an intimate, 24-hour care environment that delivers care and services to the disabled, cognitively impaired, and acutely ill and injured.
